Dietary Service Worker at The Legacy Rehab & Care Center Job Description

We are looking for a reliable dietary service worker to join our organization. You will be responsible for the preparation, presentation, delivery, service and cleanup of nutritious and healthy food to the members of our residential facility. You will have experience in food preparation, and understand the importance of following all health and sanitary precautions needed for food service. The successful candidate will ensure full compliance with nutritional standards, internal procedures and government regulations while preparing and serving foods and beverages to all residents. An understanding of quality customer service is important. Working in our facility requires compassion and strong teamwork skills are a must. Duties and Responsibilities

  •  Clean and set tables for residents
  • Set up any seasonal decorations, candles, or table cloths
  • Greet residents and assist them to their tables
  • Help cooks in preparing meals according to special dietary needs
  • Deliver prepared trays to tables or rooms
  • Assist residents with removing lids, arranging drinking straws, and locating utensils
  • Remove dirty dishes, trays, and utensils
  • Deliver used dishes, trays, and utensils to the kitchen for cleaning
  • Scrape, stack, or load dishes, utensils, glassware into the dishwasher as needed
  • Vacuum, sweep, or mop dining room
  • Remove trash; replace trash bags
  • Clean up resident spills as needed
  • Clear table as residents complete their meals
  • Follow and maintain all sanitation and safety procedures
  • Monitor and stock all food preparation stations
  • Prepare kitchen and food preparation for the next meal

Requirements and Qualifications

  •  High school degree or equivalent preferred
  • Obtain and maintain a food handler’s certification
  • Obtain and maintain CPR certification
  • Obtain and maintain DPS Fingerprint Clearance Card
  • Able to work nights, weekends, and holidays
  • Physically able to stand and move during the length of shift; able to bend and kneel through shift; able to pick up debris from the floor

Job Duties and Responsibilities

  • Serve meals to residents; help them eat and drink if necessary
  • Help bathe, groom, and dress the patient
  • Reposition and turn bedridden residents; assist those who need help walking or assistance transporting in wheelchairs
  • Keep rooms clean and sterile and change soiled bedding and bedpans
  • Take vital signs daily and record patient eating habits and bodily functions, reporting any changes to a nurse or physician

  • Work effectively with general supervision, ability to organize and manage own work load in a busy environment with some independent actions are required.

Job Requirements and Qualifications

  •  High School degree or equivalent (GED) preferred
  • Completion of state-approved CNA education and training program
  • CNA certification

  • Knowledge of medical terminology
  • High level of patience, empathy, and compassion

The cook will prepare meals and follow establishment recipes. Duties include preparing ingredients, adhering to the facility menu, and following food health and safety procedures. Cook, clean, assist other cooks and staff and ensure full compliance with nutritional standards, internal procedures and government regulations while preparing foods for residents in a fast-paced environment. Work may include operating a grill, oven, or fryer. Previous experience in a team environment and various cooking methods preferred. An understanding of quality customer service is important. Cook Duties and Responsibilities

Setting up, cleaning, and organizing work stations
Cleans, maintains, and services required equipment, appliances and tools used during performance of job duties while maintaining a safe, clean working environment.
Preparing ingredients for the shift; washing vegetables, chopping, seasoning meat, etc.
Preparing oven, utensils, and grill for shift
Cooking order according to food health and safety standards
Recommending ideas for specials or seasonal dishes
Determines proper amounts of food and baked goods to prepare based on menus and meal headcounts.

Delivering food order to dietary service workers in a timely manner

Cook Requirements and Qualifications

Able to work in a fast-paced environment

Physical endurance to stand for an entire shift

Able to quickly memorize complex meals or multiple orders
High school degree or equivalent

Assistant Director of Nursing at The Legacy Rehab & Care Center Job Description

The Assistant Director of Nursing demonstrates commitment to resident care by supporting the Director of Nursing with clerical, administrative, planning and organization responsibilities while providing professional nursing services.  The Assistant Director of Nursing is responsible for assisting the Director of Nursing with maintaining the quality and adequacy of nursing practice; for coordinating nursing and training programs; for recognizing and assisting in solving professional, administrative and supervisory problems in an assigned area.

  • Provide “on call” services to nursing staff per schedule determined by Administrator.
  • While “on call” be available via telephone at all times and be able to report to facility within thirty (30) minutes if necessary.
  • Be knowledgeable regarding state/federal regulations and facility policies and procedures.
  • Perform audits and intervene as necessary to maintain departmental compliance with state/federal regulations and facility policies and procedures.
  • Demonstrate the ability to assess the full scope of patient needs, using the nursing process on a continuous basis.
  • Provide a safe environment for residents, visitors and employees through identifying, correcting and reporting health and safety concerns.
  • Assist the Director of Nursing in developing and implementing nursing policies and procedures.
  • Participate in the quality management program and attend all meetings.

  • Identify areas of poor performance in licensed nursing staff and provide education and disciplinary action as necessary.
  • Assist the Director of Nursing in developing the monthly staffing schedules for licensed nursing staff.
  • Assist the Director of Nursing in initiating and documenting thorough investigations on all allegations of abuse.
  • Participate in annual inspections of facility by the Department of Health Services.
  • Work with Administrator and Director of Nursing to develop a plan of correction for any deficiencies noted during survey.
  • Assist the Director of Nursing in educating department staff and implementing changes per the plan of correction.
  • Work as RN Floor Nurse, providing direct care to residents, when other coverage cannot be provided.
  • Perform all delegated tasks that are within scope of practice.
  • Absorb job responsibilities of Director of Nursing in his/her absence.

REQUIRED MINIMUM EDUCATION, EXPERIENCE AND TRANING

  • Must be a graduate of a state approved school of nursing.
  • Must have a current Arizona Registered Nursing License or a current license from a compact license state.
  • Must hold a current Arizona State Board Registered nursing license and must maintain license renewal in accordance with the standards of the State Board of Nursing.
  • Previous experience in geriatric care preferred
  • Attend mandatory nursing meetings and continuing education classes that are designated mandatory.
  • Obtain/maintain Arizona Department of Public Safety Fingerprint Card at employee’s expense.
  • Obtain/maintain CPR Certification.
  • Complete annual Tuberculosis Testing.
  • Complete required 12 hours of annual in-service training.

We are looking for a driver to transport residents to and from the facility for physician appointments, new admit pickups and personal outings as needed. As a driver you must have a safe driving record, follow all state and national driving laws, rules, and regulations. The driver is responsible for assisting with loading and unloading of residents and requires an individual that is compassionate, understanding and outgoing to give our residents the best transport experience possible. You will also maintain vehicle upkeep and cleanliness and assist with carrying of items needed during the transport. Duties and Responsibilities · Transport residents from facility to physician appointments, new admit pickups and personal outings. · Assist residents in and out of vehicle. · Physically maneuver wheelchair residents up and down ramp or automatic lift; · Secure wheelchair residents in vehicle. · Be punctual in attendance and maintain timing schedules. · Keep operation records and make simple reports. · Maintain regular checks of the vehicle and report malfunctions to management as soon as possible, assures regular maintenance of vehicle. · Provide outstanding customer service in transporting of residents. · Secure and lock vehicle. · Flexibility in transport schedule, including nights and weekends as needed. Requirements and Qualifications · Must be able to sit for long periods of time. Standing for short periods of time. · Ability to lift, lower, push, pull and retrieve objects weighing a minimum of 30 lbs frequently, sometimes up to 200 pounds or more. · Must be able to climb, stoop, kneel, bend, stretch and reach frequently.
